Confirmed to teach a 300-hour training module? This covers your YTT300 Module Instructor Agreement and the paperwork that goes with it — whether you already teach at Radiance or this is your first time. Signed fresh for each module, not on a yearly cycle.

One Form Covers It All

Everything below is collected in a single onboarding form, agreement signature included — nothing separate to print, scan, or email..

  • Signed YTT300 Module Instructor Agreement — specific to this module. You sign right in the form.
  • Current Certificate of Liability Insurance — naming Radiance Yoga as Additional Insured. Required for every module. Upload a copy, or check a box confirming you’re covered and give us your policy’s renewal date.
  • Current CPR/AED certification — required only if this module is movement-based; lecture-based modules are exempt. Upload a copy, or check a box confirming you’re current and give us its expiration date.
  • W-9 & direct deposit — already an active Radiance teacher with these on file? Skip this section. First time teaching here in any capacity? Add them here so you can get paid for the module.

CPR/AED certifications and insurance policies rarely renew on the same schedule as your teacher agreement — the form doesn’t force a document upload if you’re already current. Confirm you’re covered, tell us the date, and we’ll follow up closer to when it’s actually due.
