Welcome to Radiance! One form covers everything you need to get set up to teach — most new teachers finish it in under 15 minutes.
One Form Covers It All
Everything below is collected in a single onboarding form, agreement signature included — nothing separate to print, scan, or email..
- Signed agreement – Independent Contractor Agreement, or the YTT300 Module Instructor Agreement if you’re teaching 300-hour training modules. You sign right in the form.
- W-9
- Direct deposit details – bank account & routing number, for payments by the 15th of each month.
- Current CPR/AED certification – upload a copy of your card, or check a box confirming you’re current and give us your certification’s expiration date so we can follow up before it lapses.
- Current Certificate of Liability Insurance – naming Radiance Yoga as Additional Insured. Upload a copy, or check a box confirming you’re covered and give us your policy’s renewal date
CPR/AED certifications and insurance policies rarely renew on the same schedule as your teacher agreement — the form doesn’t force a document upload if you’re already current. Confirm you’re covered, tell us the date, and we’ll follow up closer to when it’s actually due.
