Renew Your Teacher Agreement
Getting Started
Agreements run February 1 through January 31, so this comes around every year. One form covers re-signing, confirming your insurance and CPR/AED, and updating your W-9 or direct deposit if anything’s changed.
One Form Covers It All
Everything below is collected in a single onboarding form, agreement signature included — nothing separate to print, scan, or email..
- Re-signed agreement — Independent Contractor Agreement, or the YTT300 Module Instructor Agreement, for the coming year. Terms occasionally change year to year, so give it a read before signing.
- Current Certificate of Liability Insurance — naming Radiance Yoga as Additional Insured. Upload a current copy, or check a box confirming you’re still covered and give us your policy’s renewal date.
- Current CPR/AED certification — certifications typically run two years, so yours won’t always line up with your agreement year. Upload your current card, or check a box confirming you’re current and give us its expiration date.
- W-9 & direct deposit
- — stay on file automatically. The form has an optional section to update them if your bank, address, or name has changed.
CPR/AED certifications and insurance policies rarely renew on the same schedule as your teacher agreement – the form doesn’t force a document upload if you’re already current. Confirm you’re covered, tell us the date, and we’ll follow up closer to when it’s actually due.
